Information and Support for Healthcare Professionals

The Social Welfare and Housing Workstream is designed to help health and social care professionals address non-clinical barriers to discharge, such as unsuitable housing or financial challenges faced by families. We are working on collaborating with Integrated Care Boards, Local Housing Authorities, and Charities to create clear pathways, useful resources and educational opportunities to address social welfare and housing challenges that affect LTV patients.

We offer:

• Advice and support for patient-related welfare and housing queries
• Development of resources and self-help tools for professionals and families
