In this Discharge Forum, we will be introducing our new Discharge-Hub microsite and poster, as well as the relaunch of our Winter Welfare Hub.

We will be joined by Leah Moffat and Eleanor Wilson from the Social Work team in Great Ormond Street Hospital.

After Leah and Eleanor’s presentation, we’ll have time for open discussion. This is a chance to share your experiences of hospital discharges – whether things went smoothly or there were challenges -especially where Social Work or Children’s Services were involved. We’d love to hear what you learned, and it’s also a great opportunity to listen to and learn from your peers’ experiences, or to ask questions.
