Discharge Hub
What is the Discharge Hub?
The Discharge Hub is a visual roadmap showing each stage of the LTV discharge pathway – from ethical decision-making and initiation, through to planning, community coordination, and long-term follow-up plans for readmission.
At each step, users can find:
- Chain icons đź”— linking directly to key resources such as templates, checklists, and guidance documents.
- Information icon ℹ️ leading to further detail about that part of the pathway.
There’s also a dedicated section on the transition from children’s to adult services, offering tailored advice and materials to support young people and families through this process.
Who is it for?
The Hub has been designed to support all professionals involved in hospital discharge and long-term ventilation care, including:
- Discharge coordinators and complex needs nurses
- Clinical nurse specialists and ward managers
- Patient flow coordinators and doctors
- It also contains valuable materials for occupational therapists, social workers, family liaison/support workers, and welfare advisors.
How to use the Hub
Clinical teams can use the Hub in several ways:
