DESCRIPTION:In this Discharge Forum\, we will be introducing our new Discharge-Hub microsite and poster\, as well as the relaunch of our Winter Welfare Hub. \nWe will be joined by Leah Moffat and Eleanor Wilson from the Social Work team in Great Ormond Street Hospital. \nAfter Leah and Eleanor’s presentation\, we’ll have time for open discussion. This is a chance to share your experiences of hospital discharges – whether things went smoothly or there were challenges -especially where Social Work or Children’s Services were involved.
